This study aimed to analyze the presence of infrastructure and adequate work processes in primary health care (PHC) for the diagnosis, monitoring, and treatment of tuberculosis (TB) in Brazil from 2012 to 2018. This is a temporal trend study carried out with data from basic health units (BHU) evaluated in the cycles I (2012), II (2014), and III (2018) of the Brazilian National Program for Improvement of Access and Quality of Basic Care (PMAQ-AB). Variance-weighted least-squares regression was used to estimate annual changes, in percentage points, of the infrastructure and adequate work process of TB in relation to the macroregion, municipality size, Municipal Human Development Index, and Family Health Strategy coverage. The sample consisted of 13,842 BHU and 17,202 health teams in cycle I; 24,055 BHU and 29,778 teams in cycle II; and 28,939 BHU and 37,350 teams in cycle III. There was a gradual improvement in the proportion of infrastructure and work process for TB care over the three cycles of the PMAQ-AB, but none of the sites is fully adequate. The greatest trend of adequate infrastructure was observed in the South Region, and in 2018, 76.5% of the UBS had all the instruments for TB care. The greatest trend of adequate work process was in the North Region, and in 2018, 50.8% of the teams had all the items for TB care. The Brazilian National Program for Tuberculosis Control and the PMAQ-AB have contributed to these advances, but there is still a need to promote public policies that ensure the continuous improvement of TB care in PHC, the effectiveness of TB control and prevention measures.


O objetivo do estudo foi analisar a presença de infraestrutura e processo de trabalho adequados na atenção primária à saúde (APS) para o diagnóstico, o monitoramento e o tratamento da tuberculose (TB) no Brasil de 2012 a 2018. Estudo de tendência temporal realizado com dados das unidades básicas de saúde (UBS) avaliadas nos ciclos I (2012), II (2014) e III (2018) do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB). Foi empregada a regressão de mínimos quadrados ponderada por variância para estimar as mudanças anuais, em pontos percentuais da infraestrutura e processo de trabalho adequado da TB em relação à macrorregião, ao porte do município e ao Índice Municipal de Desenvolvimento Humano e cobertura da Estratégia Saúde da Família. A amostra foi composta por 13.842 UBS e 17.202 equipes de saúde no ciclo I, 24.055 UBS e 29.778 equipes no II e 28.939 UBS e 37.350 equipes no III. Observou-se melhora gradual na proporção de infraestrutura e processo de trabalho ao atendimento da TB ao longo dos três ciclos do PMAQ-AB; contudo, nenhum local está integralmente adequado. A maior tendência de infraestrutura adequada foi verificada na Região Sul e no ano de 2018, em que 76,5% das UBS tinham todos os instrumentos para o cuidado à TB. A maior tendência de processo de trabalho adequado foi na Região Norte e no ano de 2018, em que 50,8% das equipes tinham a totalidade de itens para o cuidado à TB. O Programa Nacional de Controle da Tuberculose e o PMAQ-AB contribuíram para tais avanços, mas ainda é necessário o fomento de políticas públicas que garantam a melhoria contínua da assistência à TB na APS e a eficácia das medidas de controle e prevenção da doença.

Estudio de tendencia temporal realizado con datos de las unidades básicas de salud (UBS), evaluadas en los ciclos I (2012), II (2014) y III (2018) del Programa Nacional de Mejoría de Acceso y Calidad de la Atención Básica (PMAQ-AB). Se utilizó la regresión de mínimos cuadrados ponderada por varianza para estimar los cambios anuales, en puntos porcentuales de la infraestructura y el proceso de trabajo adecuado de la TB en relación con la macrorregión, el tamaño del municipio, el Índice Municipal de Desarrollo Humano y la cobertura de la Estrategia de Salud de la Familia. La muestra se compuso de 13.842 UBS y 17.202 equipos de salud en el ciclo I, 24.055 UBS y 29.778 equipos en el ciclo II y 28.939 UBS y 37.350 equipos en el ciclo III. Se observó una mejoría gradual en la proporción de infraestructura y proceso de trabajo en la atención de la TB a lo largo de los tres ciclos del PMAQ-AB, sin embargo, ningún local está completamente adecuado. Se verificó la mayor tendencia de infraestructura adecuada en la Región Sur y, en 2018, el 76,5% de las UBS tenían todas las herramientas para el cuidado de la TB. La Región Norte tuvo la mayor tendencia de proceso de trabajo adecuado y, en 2018, el 50,8% de los equipos tenían todo lo necesario para el cuidado de la TB. El Programa Nacional de Control de la Tuberculosis y el PMQA-AB contribuyeron para estos avances, pero aún es necesario promover políticas públicas que aseguren la mejoría continua de la asistencia de la TB en la APS y la eficacia de las medidas de control y prevención de la enfermedad.

It is necessary to understand the relationship between different models of exercise periodization and the reduction of cardiovascular risk in adults with obesity. The aim of this study was to verify the effect of two periodization models of combined training on the cardiovascular risk of adults with obesity of both sexes. A randomized clinical trial was conducted with adults of both sexes with obesity. They were divided into three groups: control group (CG), non-periodized combined training group (NG), and combined training group with linear periodization (PG). The NG and PG groups underwent physical exercise training regimen for 16 weeks, in three weekly sessions of 60 min each, with the volume and intensity equalized. Cardiovascular risk was measured by the overall Framingham risk score (FRS). Generalized estimation equations and individual responsiveness analyses were used, stratified by sex. A statistically significant reduction in FRS was observed only in men of the NG (pre: 2.50 ± 0.56; post: 0.50 ± 1.02; p-value = 0.001). There was no statistically significant intervention effect on the women's cardiovascular risk. It was found that, regardless of sex, subjects in the control group mostly presented results of increased cardiovascular risk. In contrast, those belonging to the exercise groups, if not reduced, at least stabilized the chances of suffering a cardiovascular event in the next ten years after 16 weeks of combined training. Sixteen weeks of non-periodized combined training were sufficient to reduce cardiovascular risk in men with obesity. Both periodization models were important to stabilize the risk of developing a cardiovascular disease in the next 10 years.

OBJECTIVE: To estimate prevalence, assess trends and identify factors associated with non-performance of Pap smears among postpartum women residing in Rio Grande, Southern Brazil. METHODS: Between 01/01 and 12/31 of 2007, 2010, 2013, 2016 and 2019, previously trained interviewers applied a single standardized questionnaire at the hospital to all postpartum women residing in this municipality. It was investigated from the planning of pregnancy to the immediate postpartum period. The outcome consisted of not performing a Pap smear in the last three years. The chi-square test was used to compare proportions and assess trends, and Poisson regression with robust variance adjustment in the multivariate analysis. The measure of effect was the prevalence ratio (PR). RESULTS: Although 80% of the 12,415 study participants had performed at least six prenatal consultations, 43.0% (95%CI 42.1-43.9%) had not been screened in the period. This proportion ranged from 64.0% (62.1-65.8%) to 27.9% (26.1-29.6%). The adjusted analysis showed a higher PR for not performing Pap smears among younger puerperal women, living without a partner, with black skin color, lower schooling, and family income, who did not have paid work during pregnancy or planned pregnancy, who attended fewer prenatal consultations. smoked during pregnancy and were not being treated for any illness. CONCLUSION: Despite the improvement in coverage, the observed rate of non-performance of Pap smears is still high. Women most likely to have cervical cancer were those who had the highest PR for not having this test.

Health counseling is a prevention and health promotion action, especially in the context of a pandemic, for both preventing disease and maintaining health. Inequalities may affect receipt of health counseling. The aim was to provide an overview of the prevalence of receiving counseling and to analyze income inequality in the receipt of health counseling. METHODS: This was a cross-sectional telephone survey study with individuals aged 18 years or older with diagnosis of symptomatic COVID-19 using RT-PCR testing between December 2020 and March 2021. They were asked about receipt of health counseling. Inequalities were assessed using the Slope Index of Inequality (SII) and Concentration Index (CIX) measures. We used the Chi-square test to assess the distribution of outcomes according to income. Adjusted analyses were performed using Poisson regression with robust variance adjustment. RESULTS: A total of 2919 individuals were interviewed. Low prevalence of health counseling by healthcare practitioner was found. Participants with higher incomes were 30% more likely to receive more counseling. CONCLUSIONS: These results serve as a basis for aggregating public health promotion policies, in addition to reinforcing health counseling as a multidisciplinary team mission to promote greater health equity.

BACKGROUND: Evidence on inequalities in the health services use is important for public policy formulation, even more so in a pandemic context. The aim of this study was to evaluate socioeconomic inequities in the specialized health use services according to health insurance and income, following COVID-19 in individuals residing in Southern Brazil. METHODS: This was a cross-sectional telephone survey with individuals aged 18 years or older diagnosed with symptomatic COVID-19 using the RT-PCR test between December 2020 and March 2021. Questions were asked about attendance at a health care facility following COVID-19, the facilities used, health insurance and income. Inequalities were assessed by the following measures: Slope Index of Inequality (SII) and Concentration Index (CIX). Adjusted analyses were performed using Poisson regression with robust variance adjustment using the Stata 16.1 statistical package. RESULTS: 2,919 people (76.4% of those eligible) were interviewed. Of these, 24.7% (95%CI 23.2; 36.3) used at least one specialized health service and 20.3% (95%CI 18.9; 21.8) had at least one consultation with specialist doctors after diagnosis of COVID-19. Individuals with health insurance were more likely to use specialized services. The probability of using specialized services was up to three times higher among the richest compared to the poorest. CONCLUSIONS: There are socioeconomic inequalities in the specialized services use by individuals following COVID-19 in the far south of Brazil. It is necessary to reduce the difficulty in accessing and using specialized services and to extrapolate the logic that purchasing power transposes health needs. The strengthening of the public health system is essential to guarantee the population's right to health.

O objetivo desta pesquisa foi descrever e analisar a série temporal da inserção de profissionais de educação física (PEF) no Sistema Único de Saúde (SUS) no período de 2007 a 2021, bem como apresentar a distribuição por regiões e estados de profissionais no território brasileiro. Trata-se de um estudo descritivo e de série temporal que utilizou dados secundários oriundos do sistema de consultas do Cadastro Nacional dos Estabelecimentos de Saúde (CNES). Analisou-se o quantitativo de PEF no SUS nas cinco regiões e nos vinte e seis estados e no Distrito Federal entre 2007 e 2021 e reali-zou-se a análise temporal para verificar a distribuição dos PEF nas regiões brasileiras. Os resultados apontam para um crescimento no número de PEF entre 2007 (n = 22) e 2020 (n = 7.560) em todo o país. Em 2021 observa-se uma redução de 2,8% no quantitativo de PEF. Entre as regiões, os maio-res quantitativos foram observados nas regiões Nordeste e Sudeste, enquanto nos estados a maior concentração foi em Minas Gerais, seguido de São Paulo, Pernambuco, Bahia e Rio Grande do Sul. Observou-se uma tendência crescente de PEF nas regiões Norte e Sul. Conclui-se que o quantitativo de PEF atuantes no SUS teve um crescimento expressivo durante o período analisado, sendo fruto das políticas públicas e programas voltados à promoção da atividade física constituídos ao longo do tempo. Apesar disso, há importantes disparidades geográficas que não devem ser desconsideradas, reforçando a necessidade de estratégias coletivas e investimentos em políticas públicas que fomentem a inserção dos PEF na rede assistencial e de fortalecimento do SUS


The aim of this study was to describe and analyze the time series of physical education professionals (PEF in Portuguese) in the Unified Health System (SUS in Portuguese) from 2007 to 2021, as well as to present their distribution by Brazilian regions and states. This is a time series study which used secondary data from the Consultation System of the National Registry of Health Establishments (CNES in Portuguese). We analyzed the number of PEF in the country, in the five Regions and in the twenty-six States and the Fed-eral District between 2007 and 2021 and the temporal analysis was carried out to verify the distribution of PEF in the Brazilian regions. The results point to an increase in the number of PEF between 2007 (n = 22) and 2020 (n = 7560). In 2021, there is a 2.8% reduction in the amount of PEF. Among the regions, the highest numbers were observed in the Northeast and Southeast regions, while in the states the highest con-centration was in Minas Gerais, followed by São Paulo, Pernambuco, Bahia and Rio Grande do Sul. There was an increasing trend in the distribution of PEF in the North and South regions. The number of PEF working in the SUS had an expressive growth during the analyzed period, being the result of public policies and programs aimed at promoting physical activity constituted over time. Despite this, there are important geographic disparities that should not be disregarded, reinforcing the need for collective strategies and financ-ing in public policies that encourage the insertion of PEFs in the care network and the strengthening of SUS

RESUMO Objetivo: Estimar a prevalência, avaliar a tendência e identificar fatores associados à não realização de citopatológico de colo uterino (CP) entre puérperas em Rio Grande (RS). Métodos: Entre 1o de janeiro e 31 de dezembro de 2007, 2010, 2013, 2016 e 2019, entrevistadores previamente treinados aplicaram, ainda no hospital, questionário único e padronizado a todas as puérperas residentes neste município que tiveram filho nos hospitais locais. Investigou-se desde o planejamento da gravidez até o pós-parto imediato. O desfecho foi constituído pela não realização de CP nos últimos três anos. Utilizou-se teste χ² para comparar proporções e avaliar tendência e regressão de Poisson com ajuste da variância robusta na análise multivariável. A medida de efeito utilizada foi a razão de prevalências (RP). Resultados: Apesar de 80% das 12.415 participantes do estudo terem realizado 6+ consultas de pré-natal, 43,0% (intervalo de confiança de 95% — IC95% 42,1-43,9%) não realizaram CP no período. Essa proporção variou de 64,0% (62,1-65,8%) a 27,9% (26,1-29,6%). Após a análise ajustada, puérperas de menor idade, cor da pele preta, sem companheiro, de menor escolaridade e renda familiar, que não exerciam trabalho remunerado, não planejaram a gravidez, realizaram menor número de consultas de pré-natal, fumaram na gravidez e não fizeram tratamento para alguma doença mostraram RP significativamente maior à não realização de CP em relação às demais. Conclusão: Apesar de melhora na cobertura, a taxa observada de não realização de CP ainda é elevada. Mulheres mais propensas a ter câncer de colo uterino foram as que apresentaram as maiores RP à não realização desse exame.
